DOG ADOPTION APPLICATION

You must be 21 years of age to adopt a dog or cat.

Some of the following fields are "required" - they are marked with a red asterisk - you must complete these fields in order to submit application.

By submitting this application you give FFAR permission to verify the information you provide and you agree to our Privacy Policy.

Our adoption process for dogs does include a home visit for approved applications by one of our volunteers so we are only able to adopt to homes within reasonable driving distance of dog's foster home which is usually in Greater Dallas/North Texas area.

Once we receive your application one of our volunteers will contact you, usually within 24 to 48 hours.

Our adoption fee for dogs is $250 - that includes spay/neuter, age appropriate vaccinations, deworming, heartworm testing (and treatment if necessary), microchip and registration.  We also provide any medical treatment as required

All questions must be answered truthfully and completely.  In the event of falsification, FFAR has the right to reclaim the dog even after adoption.
